What to Bring
Please be prepared prior to arriving to our test-site with the following items:

Insurance card, if applicable. (Insurance will be billed with no copay.)

Current ID (parents can utilize own ID for minors).

Mobile Device- Smart phone preferred.

If English is a second language, please bring a translator to assist in the testing process.
